Long Distance Fertility Consultation - Advanced Fertility Center of Chicago

This service is appropriate for any of the following situations:

1. Couples that have not yet seen a doctor for infertility.
2. Couples that have seen a general gynecologist for infertility, but not a specialist.
3. Couples that have seen a specialist, but want a second opinion about their case.
4. Couples from out of town that want Dr. Sherbahn to review their situation and discuss it with them before beginning treatment with us.
5. Couples from out of our area that want to do IVF or egg donation here.

• Dr. Sherbahn will review your records and the information you provide on our questionnaire
• A phone and GoToMeeting (web-based meeting) will be set up with Dr. Sherbahn
• The review and discussion lasts 30-60 minutes
• Cost is $250

Here is how it works:

1. Print the long distance questionnaire (Adobe PDF file) - or call our office and request that we mail (or email) it to you.
2. Fill out the questionnaire and fax, mail, or email it back to us. Also send us a copy of all relevant previous medical records.
3. Give us your credit card information or check for $250 (payment is non-refundable if you cancel the appointment).
4. Call, or email us to schedule a time for the consultation.
5. Dr. Sherbahn will review your information and discuss your case at the scheduled time.
