Long distance consultation with Dr. Sherbahn

This service is appropriate for any of the following situations:

1. Couples that have not yet seen any physician for their infertility.

2. Couples that have seen a general gynecologist for infertility, but have not yet seen a specialist.

3. Couples that have seen a specialist, but want a second opinion regarding their case.

4. Couples from out of town that want Dr. Sherbahn to review their situation and discuss it with them prior to beginning treatment in our center.

Dr. Sherbahn will review your records and the information you provide on our questionnaire form. A telephone consultation with Dr. Sherbahn will then be arranged to discuss your situation. This discussion usually lasts about 30 minutes, but may be shorter or longer depending on the specifics of your case. The charge for this is $200.

Here is how it works:

1. Print the long distance questionnaire (Adobe PDF file) - or call our office and request that we mail it to you.

2. Fill out the questionnaire and fax or mail it back to us. Mail us a copy of all relevant previous medical records.

3. Give us your credit card information or check for $200 (payment is non-refundable if you cancel the appointment). Call us and schedule a time for the phone consultation. 

4. Dr. Sherbahn will review your information, and discuss your case at the designated time.

Couples having these consultations are not considered patients of Dr. Sherbahn's. He is rendering a professional opinion regarding your case but is not establishing medical care unless you come to our office, have a physical examination and become officially registered as a patient in the practice.
